A RESOLUTION OF THE BOARD OF DIRECTORS OF THE

MONTEREY PENINSULA WATER MANAGEMENT DISTRICT REGARDING

BOARD MEMBER PARTICIPATION IN DISTRICT HEALTH PLANS

WHEREAS, the Monterey Peninsula Water Management District offers health insurance coverage for its regular employees; and

WHEREAS, members of the Monterey Peninsula Water Management District Board of Directors are eligible for coverage under these plans

NOW THEREFORE, BE IT RESOLVED:

1. Members of the Monterey Peninsula Water Management District Board of Directors may elect to enroll themselves and eligible dependents in the Laborers Special Plan III, or another voluntary plan that allows the reimbursement of premium costs by Directors to the District, in order to receive health coverage for medical, dental, and vision care.

2. Premium costs incurred by the District for each participating Director and/or dependents should be reimbursed to the District on or before the 5^th day of each month of coverage.

3. Enrollment shall be terminated in the event that premiums become more than 30 days delinquent or the District no longer has a participation agreement with the Laborers Health and Welfare Plan of Northern California or another voluntary health plan that allows the reimbursement of premium costs by Directors to the District.
